AM Welsh Locums Limited ('we' or 'us') collect, use and are responsible for certain personal information about you. We collect the following personal information that you provide to us:
Your name, your email address, your phone number, your GMC/NMC number if appropriate, what practices you are associated with (if a practice manager). We keep records of what shifts you have made or accepted offers for, including the times and rates agreed. Optionally your CV, your Performer's List and MDO documentation, and details of your work calendar if you have chosen to upload these,
Some examples of when we collect this information include:

 b) Personal information you provide about third parties
If you give us information about another person, you confirm that the other person has appointed you to act on their behalf and agreed that you:
shall consent on their behalf to the processing of their personal data; shall receive any data protection notices on their behalf; and shall consent on their behalf to the transfer of their personal data abroad.
 c) Monitoring and recording communications
We may monitor communications such as emails and telephone calls for the following purposes:

Practices (email addresses), and, if you have chosen to share: phone number, details on your practice profile including CV, MDO documents, etc.

Name, email address, GMC/NMC number if appropriate, Practice affiliation (for practice managers)
This is to enable us to do the following:

 How long your personal information will be kept
We will hold your personal information for the following periods:
Name, email address, phone number (if provided) - until the account is closed Records of offers/shifts - indefinitely - to allow discussion of any issues or disputes that may arise in the future Records of sent emails - 12 months maximum
These periods are no longer than necessary in each case.
